Chase Blane Howard, 8 days old, went home to his heavenly Father on March 21, 2014.

Born March 13, 2014, he was the beloved son of Samantha Mariah Unsworth-Howard and Donald Bruce Howard, Jr.

Besides his parents, Chase is survived by his sister Shelby L. Howard.

Funeral services will be at Voss Chapel on Wednesday, March 26, at 6:00 PM.

Family and friends may call at Voss Chapel on Wednesday from 4:00 PM until time of service.

Memorials may be written to the Peyton Manning Children’s Hospital, Indianapolis.
